I wish to hire David E. Shapiro, trading as Safety First ECC&SE, to inspect the wiring at the
address,__________________________________________________________, Washington,
D.C., work that I performed via Electrical Permit #_______________________________________.
I have applied for and received permission from DCRA to have him perform this inspection.
If he notes any violation(s), I recognize that I have three choices: first, I can convince him that the work is not in violation, by showing him the applicable text in my Code book; second, if I have the materials needed, and the time, I can correct the violation(s) while he waits, paying for his extra time if this runs into over an hour of his time on-site; or, third, I can accept a red tag and pay him for the inspection, bringing him or another inspector back later. I appreciate the fact that no inspection is guaranteed to find all violations or hazards that may be present. I also recognize that DCRA reserves the right to come behind any Third Party Agency, and their employee may sample different items and make different judgement calls.
I understand that the normal* base fee consists of $84 an hour for round-trip travel time and $108 an hour for basic inspection, with a one hour minimum of inspection time charged. Basic inspection includes a verbal report of his findings, a red or green tag, and a report to DCRA.

* There are additional costs in two circumstances. (1) Where what's required is not just a basic inspection but formal consultation, involving a written report or a deposition, this costs an extra $30 an hour (not for the travel, though). (2) Where what's required includes abnormal scheduling, this costs more per hour for all my time, both the inspection and travel, as follows:
